Job Description:

PCP Coordinator Family Service Association is a comprehensive private, non-profit social service agency dedicated to the development and implementation of services designed to provide strength and support to individuals and families throughout southeastern Massachusetts. We have an exciting opportunity in a new program initiative at Family Service Association. We are seeking a full time (35 hours per week), Monday through Friday, 9:00 a.m. - 5:00 p.m., PCP Coordinator in our Long Term Support Service Community Program (LTSS CP). The Long Term Services and Supports (LTSS) Community Partners (CP) PCP Coordinator is responsible for assisting the program to strengthen and improve integration with PCPs and their practices, as well as other entities associated with the program?s Accountable Care Organizations (ACO) and Managed Care Organization (MCO) partners. This position is responsible for fostering and maintaining relationships with Health Centers, Practices, Hospitals, ACO/MCO points of contacts. Duties and Responsibilities: + Respect the confidential nature of all program services and communications therewith. + Foster and maintain collaborative relationships with PCPs , their practices, Health Centers, ACO / MCO affiliates, and ACO / MCO point of contacts. + Responsible for development of marketing materials, including but not limited to Letter of Introduction to PCP / practice, contact information and list of services provided, etc. for inclusion in marketing packet. Creation of slide decks and other materials for presentation. + Marketing and education of PCP / practices, health centers, etc. in conjunction with Outreach Care Coordinator. + Development of comprehensive of PCP / practices spreadsheets for tracking purposes. + Collaboration with Data Analyst to monitor effectiveness of marketing and relationship building on PCP signature rates, referrals, etc. + Making reminder calls to PCP / practices, and escalations to the ACO / MCO point of contact as appropriate to ensure LTSS Care Plan is signed by the Primary Care Provider within required time frames. Bi-directional communication with Administrative Assistant regarding same. + Maintain all documentation required for the delivery of LTSS Community Partner services. + Attend regular supervisory meetings both on a team and individual basis as required + Participation in on-going CQI activities. + Respond to reasonable expectations of the Agency and/or supervisor. Education and Experience: Must have a minimum of a Bachelor?s Degree in social work, human services, nursing, psychology, sociology, or a related field; OR an Associate?s Degree and at least one year of experience in the field. Must have strong written and oral communication skills and demonstrated computer skills. Prior experience in marketing or customer services preferred.
