Job Description:

Job Summary: Under the direction of the Clinical Manager and within established agency policy and procedure, the Physical Therapist provides a variety of skilled physical therapy services in the home care setting. Provides therapy to patients and families in their homes, which supports the total person. This is accomplished within an interdisciplinary plan of care. Works collaboratively with the Home Care Aide, Social Worker, RN, LPN, P.T.A., O.T., C.O.T.A., S.L.P., and office support staff to coordinate patient care utilizing community resources as needed. Participates in staff education. Provides consultation regarding assessment, planning and implementation of care. Coordinates activities with other community, agency and/or department staff and carries out special activities on assignment.Responsibilities:Provides superior customer service to internal and external clients, customers and patientsExercises primary responsibility for a patient caseload, as assigned by Manager; functions as a case manager when necessaryProvides assessments and customized treatment programs to patients in their homes, with the goal of improving mobility, restoring function, relieving pain, and preventing or limiting permanent disability.Responsible for reviewing all admission OASIS data and discussing with primary nurse if therapy is not primaryAccurately, promptly and thoroughly documents patient care observations, interventions and evaluationsRestores, maintains and promotes overall fitness and healthAssumes accountability for own professional practice in achieving patient outcomesAttends and participates in multidisciplinary team meetings as scheduled by Clinical ManagerDirects and aids patients in active and passive exercises; performs manual therapy techniques; performs therapeutic exercise, with and without equipmentProvides guidance to family members and caregiversOrients PTA to patient POC need and monitors PTA progress in following plan of care to meet optimal patient outcomesSupervises and orients Home Care Aide to plan of care according to regulatory requirementsMonitors patient progress to ensure accomplishment of patient goals and optimum patient outcomes; initiates changes as necessary and appropriate to roleCompletes OASIS timepoint assessmentsCollaborates with PI staffSuccessfully completes OASIS competency visit annuallyParticipates in weekly case conferencingCollaborates and communicates with other health care providers to ensure quality and continuity of careOrients new patients to range of services provided by the agencyActs as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectivelyEnsures effective communication by checking voice mails twice per day and downloading PDA at least twice dailyCollaborates with scheduling department to ensure patients are visited according to treatment plan, and fulfilling any productivity expectationAssumes accountability regarding communicating changes on patient schedules to Clinical Scheduler, and works with scheduling to maintain productivity levels if scheduled patient visits drop offAttends and participates in clinical inservices, trainings, etc.Adheres to the mission of the agencyQualifications:Current licensure from the Board of Allied Health in the State of MassachusettsCurrent valid driver's licenseGraduate of an accredited college of physical therapyMust have demonstrated competence in community health or clinical patient care setting, providing direct patient care with patients for a minimum of one - three years.Must be willing to work a minimum of one weekend per month, one major holiday and one minor holiday per calendar year and as department may require. Ability to work independently in the home care settingDemonstrates responsibility for self-directionDemonstrates ability to learn and utilize computer technologyDemonstrates effective communication skills Work Environment:Must demonstrate the ability to work in a team-oriented environment.Ability to work independently in the home care setting.Must be able to lift up to 50 pounds, push/pull up to 50 pounds, stoop, kneel or crouch (if applicable)Ability to utilize technology applicable to position, including desktop computers, hand-held computers and telephones.
