Job Description:
Element Care is seeking a dedicated and compassionate Physical
Therapist to join our interdisciplinary team serving the elderly
through the PACE program. This position involves providing tailored
physical therapy to older adults, helping them maintain or improve
their physical capabilities and overall health. The ideal candidate
will work collaboratively with our healthcare team to design and
administer therapies that help our clients live as actively and
independently as possible within the comfort of their own homes.
Evaluate the physical condition of clients to diagnose mobility and
functionality issues. Develop a treatment plan tailored to each
client's needs and abilities, focusing on pain relief, healing, and
rehabilitation. Implement therapeutic interventions for pain
management and improved mobility. Educate patients and families on
safe and effective ways to incorporate exercise and self-management
techniques into daily life. Monitor and document patient progress,
modifying treatment plans as necessary. Collaborate with other
health care professionals, such as doctors, nurses, and social
workers, to ensure a holistic approach to patient care. Maintain
confidentiality and accurate health records in compliance with
healthcare laws and regulations. Strong knowledge of physical
therapy principles, theory, methods, and their application.
Excellent interpersonal skills, with the ability to motivate and
educate elderly clients and their families. Ability to handle a
caseload with varying health conditions and physical abilities.
Proficient in the use of physical therapy equipment and techniques.
Good observation skills and ability to assess clinical needs.
Current state licensure as a Physical Therapist. CPR certified.
Preferred: Geriatric Certified Specialist (GCS) through the
American Board of Physical Therapy Specialties.
