Job Description:

A Nurse Case Manager in our esteemed healthcare facility {EMPLOYER-DESC} focuses on coordinating long-term care for patients who require regular medical attention or specific therapies. The role involves assessing, planning, facilitating, and advocating for options and services through communication and available resources to promote quality, cost-effective outcomes. Assess new patients, review their needs and eligibility for treatments and monitor ongoing progress. Develop, implement, and review individualized patient care plans. Facilitate a multidisciplinary approach and coordinate care with other health care providers. Advocate for patient needs and ensure that patient rights are respected. Evaluate patient progress and update care plans accordingly. Maintain accurate documentation of patient status and case management activities. Communicate patient status and needs to family members and other stakeholders. Strong clinical knowledge particularly in case management and coordination of care. Excellent communication and interpersonal skills. Ability to work independently and make decisions related to patient care. Proficiency in healthcare IT systems relevant to case management. Strong organizational and multi-tasking skills. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current registered nurse (RN) license in the state of practice. Certification in Case Management (e.g., Commission for Case Manager Certification) preferred.
