The Director of Community Partnership Program - Care Coordination
Services promotes the Center's mission by overseeing the
coordination of services to promote positive client outcomes across
programs. This includes ensuring timely access to services,
placement of clients in the appropriate levels of care, sufficient
capacity to address client needs and effective processes to
facilitate seamless integration and client flow across service
categories and levels. The Director collaborates with other
leadership staff in the design and implementation of service
delivery processes.Specific Responsibilities
- Director of CP, Care Coordination Services oversees activities
related to facilitating client access to services; and collaborate
with all leadership positions involved in facilitating admissions
to service programs; monitor and report service access data.
- Oversee outreach and engagement strategies to refer and link
individuals and families in need of services to the Center as
appropriate; ensure timely referrals and linkages to external
services and resources for people with needs outside of our
services; provide direct supervision to Case Managers and Resource
Specialists who assist clients in obtaining services and resources
and in addressing barriers to access.
- Director of CP, Care Coordination Services will monitor
staffing levels to ensure sufficient capacity; collaborate with
other leadership staff in the recruitment, screening and selection
- Collaborate with other leadership staff in creating and
managing effective mechanisms and processes for placing clients in
the appropriate levels of care and service programs and for
facilitating efficient client flow.
- Director of CP, Care Coordination Services ensures that client
placement in levels of care and service programs complies with all
applicable contractual and funding source requirements. This
includes but is not limited to monitoring client placement and
course of treatment in Evidence-based Practices (EBP's); provide
direct supervision to the EBP specialist who assists in monitoring
and reporting data regarding EBP training and utilization.
- Collaborate with other leadership staff in the implementation
and oversight of service quality and client outcome assessments;
oversee the timely reporting of EBP utilization and outcome
- Continuously model and reinforce best practices with regard to
leadership, management and client service; promote an environment
that is conducive to a very collaborative, highly engaged and
quality-driven program workforce.

- Master's or Doctorate degree in psychology, social work,
marriage & family therapy/counseling or other closely related
field. Licensure required.
- Experience in supervisor or management role in mental health or
other closely related human service field; knowledge and experience
in health systems, non-profit organizations (e.g., Mental Health,
Children and Family Services, etc.) is preferred.From: SSTAR
